Shift: Monday-Friday; 8am- 5pm with On-Call Rotation

Interim HealthCare of Colorado Springs is offering a sign-on bonus between the date of November 1, 2022 and December 31, 2022, for full time Case Managers.

Bonus Program: $5,000.00 (paid out $1250 at 30 days following the successful completion of orientation, $1250 at 90 day Performance Review and retention bonus of $2,500 at 1 year anniversary Performance Evaluation)

We are looking for a dependable Hospice Registered Nurse (RN) Case Manager who will manage the care of a defined caseload of patients to facilitate the healthcare team’s efficient and effective achievement of patient outcomes in accordance with the patient’s plan of care.

Responsibilities of the Hospice Registered Nurse (RN) Case Manager role include:

  • Complete initial and ongoing comprehensive assessments of the patient’s needs.
  • Provide those services and/or treatments requiring substantial and specialized nursing skill.
  • Initiate appropriate preventative and rehabilitative nursing procedures.
  • Supervise paraprofessional and licensed professional team members.
  • Perform assessments and complete appropriate documentation.
  • Update plan of cares as necessary.

Minimum Education & Experience Requirements:

  • Registered Nurse (RN) with active license in Colorado.
  • Health care CPR/BLS certified.
  • Hospice or Home Health experience preferred.
  • One (1) year of accumulated experience as a registered nurse within the last five (5) years.
  • One (1) year of Hospice care experience preferred.
  • Excellent assessment and communication skills.
  • Able to work independently with minimal supervision.
