Salary Range:

Care Coordinator/Skills Trainer I: $40,809.60 Annually

Care Coordinator/Skills Trainer II: $43,305.60 Annually

Care Coordinator/Skills Trainer III: $46,009.60 Annually

Shift Available: Full-Time

Essential Duties and Responsibilities:

• Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
• Serves as a Skills Training Specialist with Adult Behavioral Health in Amarillo
• Will be responsible for conducting state approved assessments, recovery planning with adults, 18 and older with primary diagnosis of schizophrenia, bipolar disorder, and/or major depressive disorder
• This is an intensive caseload
• This position teaches and models skills that address independent community living, social, coping, problem solving, anger management, & symptom management to increase tenure in the community
• Will be trained in state-approved curriculum that emphasize evidence based practices to help improve daily functioning
• Position will organize and provide culturally competent care activities and collaborating among all participants involved with the individual’s care
• Additional duties can include: assessment & treatment planning, working with treatment team to ensure appropriate services are delivered and taking a person centered approach to help individuals identify strategies and objectives
• Will provide services in both home and community based settings. This position will spend at least half of his/her working time directly working with the individual
• Position will assist & collaborate with the interdisciplinary Behavioral Health Team in promoting skills development and community stability
• Adheres to TPC Business Code of Conduct and reports concerns of ethical and/or business standards defined in the code if compromised

Required qualifications to be successful in the job:

• Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, medicine or other related human services field for QMHP credentialing
• Degree in social work or psychology preferred
• For Care Coordinator/Skills Trainer I, no experience required
• For Care Coordinator/Skills Trainer II, one (1) year experience of behavioral health case management
• For Care Coordinator/Skills Trainer III, two (2) years experience of behavioral health case management
• Graduate work in a related field may be substituted for up to one (1) year of experience
• Must have a valid Texas driver’s license and be insurable under agency vehicle policy
• Must be 18 years of age or older
