Seeking: Foster care families needed for adult individuals with IDD, prefer families with no small children or teenagers in the home as our individuals prefer to have all of the parents' attention. This is a daily rate position based on the level of client care.

Duty Hours: 24 hours a day, 7 days per week providing comprehensive services which provide training, supports, and supervision to the consumer with intellectual/developmental disabilities within a private home setting and in the community. The AFL provider has 24 hours/7 days a week responsibility for the consumer served and is responsible for supervision, monitoring and support of the consumer when the consumer is not engaged in other services.

It is the responsibility of the AFL provider to care for the consumer if the consumer is sick or is suspended from a day program due to his/her behavior. If AFL provider is contacted and informed that the consumer is not feeling well or that the consumer’s behavior is not acceptable, it is the AFL provider’s responsibility to come and get the consumer in a timely manner.

A Caring Heart’s AFL providers often provide day program services to other consumers while their AFL consumer is attending a day program or school. AFL providers are permitted to provide services to other consumers but their primary responsibility is to their AFL consumer. Requirements:

• Must be at least 21 years old.
• Able to effectively read, write, and communicate verbally in English, understand instructions and perform record keeping.
• Must have a reliable vehicle and safe driving record.
• Must have a valid drivers license and state minimum auto insurance.
• Background checks will be performed on all applicants.
• Not listed in the North Carolina Health Care Abuse registry.
• High school diploma or equivalent.
• Must have basic computer skills.
• May be required to pass a drug screen and must have no evidence of a communicable disease

Nice-to-have:

• Bi-lingual (English and Spanish) is a plus.

About This Position:

• AFL or Alternative Family Living is a type of Residential Supports available to NC Innovations consumers.
• An AFL provider is an individual or couple who chooses to provide Residential Supports in their home. The AFL site (home) must be the primary residence of the AFL provider. AFL is a comprehensive service individually designed to meet the needs of the consumer.
• An AFL provider receives payment for the services and support provided to the consumer. AFL providers also receive reimbursement for room and board expenses of the consumer through a separate funding source.
• Habilitative services focus on the individual's ability to reside as independently as possible in the community. The individual is assisted with acquiring, improving and retaining skills in self-help, general household management, meal preparation, personal finance management, socialization and other adaptive areas.
• Assistance in activities of daily living is provided per the individual's needs. Full participation in home life and community are also encouraged and supported. AFL provider transports the individual to and from the residence to community destinations.
