We have openings in our Sebastopol, Occidental, and Guerneville (California) clinics.

Join our dynamic integrated health care team and work at a Federally Qualified Health Care (FQHC) that prides itself on innovative and patient-centered care. You can hone your skills with opportunities to work with specific populations including our teen clinic, on-site school counseling at local elementary, middle, high-schools, gender expansion services, geriatric-focused services, HIV clinic, homelessness, Medically-Assisted Treatment, and co-facilitated group therapy. Our patients present with a broad range of issues giving our associates an opportunity to work with a variety of presenting problems. In this setting, therapy is provided to children, adults, couples and families.

  • Individual/Triadic/Group Supervision hours towards licensure from licensed supervisors (LMFT, LCSW)
  • We are dedicated to high quality trainings on a regular basis

As an integral part of the Primary Care Team, you will:

  • Work with Medical, Behavioral Health and other staff to provide assessment, intervention, and brief and ongoing behavioral health services to patients with medical and behavioral health concerns within the primary care setting.
  • Participate in care coordination, patient education, and information and referral.
  • Provide facilitation of groups for individuals with similar diagnosis and symptoms.
  • Provide crisis assessment and intervention and be available for warm hand-offs from primary care staff.
  • Focus upon prevention and early intervention from a multi-generational approach with a focus upon social determinants of health and trauma-informed practices.
  • You will invest in long-term relationships with a diverse group of patients over time to develop a trusting, healing environment for patients to thrive.

Experience & Qualifications:

  • Registered Associate MFT or MSW
  • Experience working with diverse populations providing individual, family and couple’s counseling.
  • Bilingual/bicultural in Spanish is a bonus!

Physical Requirements:

  • Must be able to move freely about the office and carry light loads (up to 25 pounds).
  • Prolonged standing and occasional sitting required.
  • Ability to work with office equipment, including computers.
