JOB DESCRIPTION SUMMARY
To establish and maintain supportive relationships with LDA Staff members, in particular those working at Daybreak: Daybreak participants;l caregivers and like services while performing related work as required.
SUPERVISION RECEIVED: Work is performed under the supervision and direction of the Daybreak Program Director and/or Director of the Lenawee County Department on Aging.
SUPERVISION EXERCISED: Will supervise Program Assistants.
ESSENTIAL DUTIES AND RESPONSIBILITIES: An employee in this class may be called upon to do any or all of the following: (These examples do not include all of the tasks which an employee may be expected to perform).

Maintain certifications (CAN or MA), CPR, Recipient Rights and Food Handlers and any others required by contract holders.
Have the ability to respond to emergency situations including performing CPR if needed and have the ability to see and hear the needs of participants in the setting.
Attend minimum of 1-2 educational trainings per year related to the participants we serve to promote personal and professional growth.
Complete yearly trainings required by program such as but not limited to hand washing, infection control and emergency preparedness.
Contribute to care plan creation as part of the regular and family care plan meetings.
Schedule program staff for direct care, providing leadership, orientation, and guidance.
Coordinate staffing schedule for Program Assistants.
Physically capable of standing for long periods of time, walking, pushing, pulling, reaching, bending and squatting. Some lifting required.
Completes assigned documentation following standards including but not limited to activity logs, nutritional logs and care plans.
Provide participant care - Participants vitals, medication administration, personal care which may include but not limited to transferring, ambulating, nail care, toileting and skin care.
Plan, lead and offer assistance with individual group activities.
Ensure participant safety by reinforcing protocols and following care plans.
Provide assistance in meeting the participants' nutritional needs such as cueing or feeding.
Distribute meals, snacks and offering fluids.
Informs appropriate staff of changes in the participant's condition and any unusual concerns.
Maintain and monitor a safe and clean environment by practicing infection control precautions.
Maintain a work environment of steady to fast paced and multitask as required.

DESIRABLE EDUCATION, EXPERIENCE and/or QUALIFICATIONS: An employee in this class, upon appointment, should have the equivalent of the following training and experience.

High school diploma. CAN or MA training preferred - med training.
Two years of experience working with older adults in a care setting providing meaningful activities.
Experience supervising staff.
Experience in preparing accurate written records and reports.
Ability to work effectively with the public and other employees.
Experience with Microsoft Office programs.
Physical Requirements
Lift 50 pounds Carry 30 pounds a distance of 20 feet several times a day. Push/Pull an average of 50 pounds of product on a cart. Stand on tiled floor 90% of the day. Reach/Bend/Squat several times an hour Ability to see and hear information from co-workers and equipment for safety. Maintain a steady work pace of steady to fast. Additional Information
