The Charak Center for Health and Wellness is looking for a Behavioral Health Nurse to join our team!

This position will be primarily assigned to our Mentor Office. On occasion, my cover at another site. Must be fully COVID-10 vaccinated.

Scheduled work hours: Typical Medina Center business hours are 9 a.m. to 5:30 p.m., Mondays through Friday. From time-to-time, however, the work situation may require an employee to expend an amount of time in excess of the normal workweek. MEDSOM team meeting on Thursday mornings at 8 a.m. via telephone.

Competencies: All AGENCY positions are expected to demonstrate competency in the areas of 1) Confidentiality; 2) Cultural Sensitivity; 3) Universal Precautions and 4) Client Rights. Additional competency standards may be required for each position based on the annual plan.

Credentials (Education/Experience/Licenses): Education- H.S. Education, college preferred. Must have an active RN or LPN license. At least two years’ experience in the nursing field of mental illness. Must have a valid Ohio driver’s license. Current credentials according to the matrix in the Ohio Revised Code 5122-29. Professional license required.

OSHA Category: This position may perform tasks that involve exposure to blood, body fluids, or tissues. All employees are offered to opportunity to receive the Hepatitis B vaccination series at the Practice’s expense.

ESSENTIAL FUNCTIONS/JOB DESCRIPTION:

-Provide nursing services during office visits for Mental Health and Addictions Recovery Services. This includes gathering health information, conducting screenings including urine drug screenings, reviewing and recording medications, taking vital signs, administering injections, preparing patients for and completing ECGs/EKGs. Documenting services in electronic medical record system (EMRS).

-Provide Nursing Case Management services as needed.

-Call in prescriptions and/or follow other doctor’s orders

-Retrieve and process prescription refill requests via designated message line. Follow-up with patients. Document information in EMRS.

-Complete urine screenings, pregnancy tests and breathalyzers

-Complete prior authorizations for prescriptions

-Coordination of all samples (entering, tracking, dispensing)

-Coordinate Long acting injectable Medications

-Monitoring medication compliance and support.

-Order medical supplies and RX pads and maintain inventory control

-Do pill minders and pill boxes, as needed

-Complete all related documentation
