Breast Care Patient Navigator/Registered Nurse Job at Auburn Community Hospital
The Breast Care Patient Navigator is responsible for coordinating the care of patients with breast helath concerns, including the promotion of breast health in a timely and accurate manner. This role will include providing education and support to patients, family, staff and caregivers. The Patient Navigator will assess and monitor patients while they are hospitalized and will assist the Physician during procedures. The patient navigation process includes consistent care coordination throughout the continuum of care and an assessment of the physical, psychological, and social needs of the patient. The Patient Navigator will provide first line direction to the staff mammographers as well as direct communication with the Reading Mammography Radiologist and Breast Radiologist performing procedures.
NYS RN license required. Breast Health Navigation Certification must be obtained within one year if not currently certified.
