PURPOSE

The Burden of Illness (BOI) Program Manager is responsible for the successful planning and execution of capturing burden of illness projects as assigned by operations and population health. Responsibilities may be moderately to highly complex and could involve hands-on work with providers, executives, vendors, or other stakeholders to successfully implement innovative care models. This position has a heavy emphasis on working closely with clinic locations in support of their goals. Must have some understanding of change management and be able to integrate with physicians and staff while also being a liaison between operations, population health and Connected Senior Care Advantage (CSCA). Must also be able to communicate effectively within and across teams, departments, and management levels. Carries out all duties while maintaining compliance and confidentiality and promoting the mission and philosophy of the organization.

ESSENTIAL FUNCTIONS

  • Supports clinic locations in the understanding and achieving of their goals by:
  • Routine clinic visits.
  • Effectively communicating the tenants of the risk model while ensuring all stakeholders know the importance of any project being implemented.
  • Operations/project management support around performance improvement opportunity.
  • Conducts quantitative and qualitative analyses to support performance improvement efforts.
  • Assists with establishment and maintenance of projects and program management methodology, standardization, and other department procedures.
  • Coordinates the planning, development, and implementation of new programs or capabilities within the clinics in partnership with population health and CSCA.
  • Demonstrates an understanding of problem solving and process improvement methodology.
  • Responsible for informing clinics of weekly reporting outcomes (i.e. MIP print reports, MAE status updates, Valuable Visits and Reassessment Rates).
  • Supports clinics and population health with outreach efforts.
  • Responsible for project management and facilitating groups of people.
  • Investigates report anomalies and collaborates with CSCA on solutions.
  • Provides excellent customer services skills, including consistently displaying awareness and sensitivity to the needs of internal and/or external clients. Proactively ensuring that these needs are met or exceeded.
  • Takes responsibility for growth including acquiring new skills, knowledge, and information.
  • Engages in excellent communication which includes listening attentively and speaking professionally.
  • Sets and completes challenging goals.
  • Contributes in organizational development through meeting attendance and committee involvement.

QUALIFICATIONS

Education and Experience

Required:

  • Bachelor's degree in related area.
  • Three (3) or more years of healthcare clinical or administrative experience.
  • Experience working in or with practice management.
  • Experience working with and leading diverse groups and matrix managed environments.

Preferred:

  • Advance degree in applicable field (MBA, MPH, MHA).
  • Working experience within an Accountable Care Organization (ACO) strongly preferred.
  • Experience with project management or otherwise facilitating groups.

Knowledge, Skills and Abilities

  • Proficient with project management tools, MS-Project or other project management software, and MS-Office applications.
  • Demonstrates attention to detail and accuracy in work product.
  • Ability to engage others, listen and adapt response to meet others’ needs.
  • Ability to align own actions with those of other team members committed to common goals.
  • Ability to perform job duties in a professional manner at all times.
  • Strong communication skills both written and verbal to work with multiple internal and external clients in a fast-paced environment
  • Ability to work with mathematical concepts such as probability and statistical inference.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Strong analytical and problem solving skills.
  • Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems.
  • Excellent computer and keyboarding skills, including knowledge of Microsoft Office programs and Windows, including ability to create and maintain documents using Microsoft Office (Word, Excel, Outlook, PowerPoint)
  • Ability to effectively prioritize and execute tasks in a high-level pressure environment is crucial while eliciting cooperation from a wide variety of sources, including upper management, peers, and subordinates.
  • Experience at working both independently and in a team-oriented, collaborative environment is essential. Must be able to diffuse tension among team members, with demonstrated ability in negotiation and conflict resolution.
  • Ability to manage competing priorities.
  • Knowledge of basic components of change management.
