Care Coordinator Job at Haymarket Center
Haymarket Center is seeking a Care Coordinator to join our DuPage County team!
The Care Coordinator will work in the Drug Program Co-located inside of DuPage County Jail linking clients to treatment and other services upon release. The Care Coordinator provides individualized and evidence-based substance use recovery services to patients identified throughout the area.
The ideal candidate will possess the following:
Bachelor of Science degree in Counseling or related field:
- Possess CADC certification;
- Two years-experience working with individuals with substance-use disorders
- Experience facilitating therapeutic groups and working with culturally diverse populations
- Valid Illinois Driver’s License required
Job Type: Full-time
Pay: $45,000.00 - $46,000.00 per year
