Care Coordinator- Bilingual Job at Children Service Center
Purpose of this Position:
Children’s Service Center is currently looking for an energetic and dependable person who works well independently and collectively as a treatment team member. If you are a health professional that is ambitious and has compassion to help manage child and adult patients care, the Care Coordinator (similar to care navigator, care management navigator, patient care coordinator, clinical team coordinator, health coordinator) position is right for you. You will also be trained to become knowledgeable with community resources and health care practices. This position has traditional working hours and will report to the Care Coordinator Supervisor or designee.
*Bilingual fluent in Spanish needed*
Responsibilities include, but are not limited to:
- Ability to work independently or in a team, in a fast paced environment
- Monitor and coordinate patients treatment plans and evaluate their progress
- Educate patients about their condition and link them with health care providers for their needs
- Knowledgeable about Health Care practice requirements and Community Resources considerations (locations, eligibility requirements and program changes)
- Ability to use good judgment skills in assessing consumer needs and being creative in seeking appropriate services
- Ability to work with external providers (i.e. county entities, hospitals, insurance companies, etc.) in the community while forming professional relationships to enhance resource availability to the client

Qualifications:
Bachelor’s Degree in Social Work, Sociology, Psychology, or related social science field
Or
Associates Degree in Social Work, Sociology, Psychology, or related social science field, with one year of relevant experience
Knowledge of the mental health and social service field preferred, but not required
Must pass Safe Crisis Management Training (physical and theory), upon hire and then annually as a condition of employment. Other requirements include:
- Driver’s License
- FBI Act 73
- Criminal History
- Act 33 Clearances
