Calling all uber-organized, people-focused, solution-driven Care Coordinators! EverCare is currently seeking a Care Coordinator to work in our Newburgh, NY location. As Care Coordinator, you will play an integral role in assisting the care team in providing high quality, value conscious care to members of our Managed Long Term Care plan. You will interface with coworkers, members, their families, caregivers, and providers to coordinate services, solve problems and help members stay as healthy as possible in their homes for as long as possible.

Minimum Qualifications:
· High school diploma or GED; associate’s degree preferred
· 3+ years of experience in a fast paced, high volume office environment; experience with Medicaid/Medicare a plus
· Enviable computer skills
· Bilingual English/Spanish preferred
