Care Coordinator - Adult Outpatient Program
Pay Rate: $26.44 to 28.85 per hour

Hours: 40 hours per week (Monday- Friday); 9am-5pm every day except Wednesday, 11:00am to 7:00pm.
Maria Sardiñas Wellness and Recovery Center serves adults with serious mental illness and co-occurring substance use disorders in an outpatient setting. Job Duties:
- Case management, linkage, and referrals to supports in the community
- Assist clients with: navigation of the mental health system; meeting their goals; and increasing their self-sufficiency
Qualifications/Skills Requirement:
- Bachelor's degree in a mental health-related field
- At least one year of related experience and/or training
- Bilingual Spanish preferred; non-bilingual candidates encouraged to apply
- Proficiency in MS Office applications, specifically Word and Excel required
- Ability to work in a fast-paced and collaborative team environment
- Ability to pass criminal/background clearance
