We are seeking a Client Care Coordinator to join our team. Coordination of care in a behavioral health setting promotes integration of services that are centered on the needs of the client and their family. Services delivered by a Care Coordinator can lead to decreased health care costs, reduction in fragmented care, and improvement in care experience and quality of life. This position will also work part-time as a Support Specialist for the office.
The primary function of this position is to be the “lynch pin of integrated care”. A Care Coordinator will be instrumental in collaboration with community, scheduling Integrated Treatment Plan (ITP) meetings, assessing client needs, providing linkage to services/resources, and evaluating satisfaction of client services.

Essential Functions* include:
- Working collaboratively with Community Partners – may include travel and working from Community sites
- Attending Community meetings and events, representing Northern Pines Mental Health Center
- Scheduling ITP meetings
- Maintaining program tracking of integrated treatment planning and communicating with treatment teams
- Completing Crisis Prevention Plan with clients prior to ITP meetings
- Updating medication list by using Dr. First
- Assisting clients with resources and referrals. This may require connecting clients to Mobile Crisis Outreach services, Emergency Services and/or other supports
- Maintain excellent communication and collaboration with all programs within Northern Pines Mental Health Center
- Follow-up on referrals made for clients
- Communicate with internal and external providers as needed to ensure continuity of care
- Provide health and wellness education to clients
- Coordinate and attend Integrated Care Team meetings to address client needs
- Attend Rule 29 meeting to facilitate client needs
- Maintain accurate and timely documentation
- Evaluate client satisfaction of services

Qualifications *include:
- High School Diploma or equivalent education
- Valid driver’s license
- Experienced in Microsoft Programs, including Excel, Word, Outlook, electronic health record, etc.
- Associates or Bachelor's Degree in the Behavioral Sciences is preferred
- Qualification as a Mental Health Practitioner per the MN Department of Human Services is preferred
- At least one year of experience working in the Behavioral or Primary Healthcare field is preferred
