The Care Coordinator role is a critical member of the Landmark Interdisciplinary Team. This role reports to a supervisor or the Director, Health Services. Responsibilities are to ensure a positive patient experience with Landmark , as well as to build good relationships with local provider groups to ensure that Landmark is a reliable and trusted partner in patient care.

This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 8:30am – 5:00pm. It may be necessary, given the business need, to work occasional overtime or weekends.

Our office is located at 2150 N 107th Street, Seattle WA. Employees will be required to work 2-3 days onsite and remainder days from home

Primary Responsibilities:

  • Regular contact with provider groups to collect clinical data.
  • Support patients in scheduling home visits.
  • Ensure that Landmark has all documentation available regarding patient conditions.
  • Support Landmark providers and Nurse Care Managers with operational and clinical tasks.
  • Handle escalated patient issues, ensuring the appropriate resources are involved.
  • Follow triage protocol when needed.
  • Other duties may be assigned.
  • The position is office-based.

Required Qualifications

  • High School Diploma / GED (or higher)
  • 1+ years of experience in a clinical environment including hospital, clinic, or physician’s office
  • 1+ years of experience in office work or customer service
  • Work in the office 2 – 3 days per week (Location: Northgate and Federal Way)
  • Understanding of fundamental health-care related terminology, programs or services
  • Intermediate skill set using Microsoft office system products - Microsoft Outlook (open, edit, create, save, and send emails), Microsoft Word (open, edit, create, save, and send documents), Microsoft Excel (open, edit, create, save, and send spreadsheets), Microsoft Teams (able to communicate, IM, and join meetings) and Microsoft SharePoint (able to take an excel spreadsheet, sort, paste to a SharePoint sight
  • Ability to work an 8-hour shift Monday-Friday from 8:30 am to 5:00 pm

Preferred Qualifications:

  • Bachelor’s Degree (or higher)
  • Experience as a Medical Assistant, Certified Nursing Assistant (experience working in a medical office setting and/or Medical Assistant), Pharm Tech, or other clinical or front office administrative role in a health care setting
  • Experience with escalated customer service issue resolution.
  • Prior experience working with triage protocol
  • Typing speed of 40+ WPM

Telecommuting Requirements:

  • Reside within Seattle Washington or 25 miles radius
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
  • Ability to keep all company sensitive documents secure (if applicable)
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
