About the Role

As a Care Manager, you will work in collaboration with the client and family in planning, implementing, and evaluating service provisions to maximize the client's success in the community. You will assist clients in navigating treatment needs and provide active care management, including linkages to community resources and social determinants of health to address specific wraparound needs. You will work with an interdisciplinary team to provide direct services to children/adolescents, and their extended support system.

Direct services include, for example, relapse prevention, daily living skills, psychoeducation, client advocacy, and crisis intervention; consult with other service providers regarding the assessment, treatment and rehabilitation needs of the client; participation in an interdisciplinary team process to ensure the provision of successful implementation of individual recovery plan goals.

Requirements

  • BA degree in mental health OR related field with two years previous experience.
Annual Salary: $48,000 to $50,000
