• Current Certifications Preferred - CNA, HHA, PCA, LTC-A (Not Required)
• Home Health, Hospice, Assisted Living, Skilled Care, & Dementia Care Experience a Plus.

Responsibilities

• Assisting clients with personal care, which may include bathing, grooming, dressing, & bathroom functions
• Following the individual care plans of clients
• Monitoring and reporting changes in health, behavior,and needs
• Companionship, medication reminders, errands, meal preparation, groceries & supplies shopping
• Transporting and escorting clients to medical or non-medical appointments
• Mobility & transfer assistance may be required, for example assisting client in and out of bed, a chair, or a wheelchair
• Ensuring the client’s home is organized according to their needs and that safety measures are in place. Providing assistance with light housekeeping
