ESSENTIAL FUNCTIONS:
1. Coordinates and monitors day-to-day operations of the Case Management and Utilization Management programs.
2. Directs the activities necessary to ensure appropriate utilization of the Hospital and its resources.
3. Maintains optimal achievable standards of patient care.
QUALIFICATIONS:
· Requires intermediate level knowledge of computers and patient care documentation in an EMR (Electronic Medical Record) environment.
- Two years of clinical experience in an acute care setting is preferred.
- One year of Case Management/Utilization Review/Discharge Planning experience are preferred.
LICENSE AND CERTIFICATIONS:
Requires clinical licensure from an accredited school (RN/LVN/LSW).
- Certification in Case Management (CCM) is preferred.
