Case Management
Master's degree in the helping field i.e. social work, counseling, psychology, or human services, must be flexible with work schedule, experience working with MH/SA population completing family intakes and, provides patient education and counseling, advises caregivers, and makes necessary referrals for other social services, minimum of one year case management experience, excellent problem –solving skills, excellent communication skills and candidates must possess good documentation, demonstrate responsibility and reliability accountability and dependability.
