We are seeking a resourced Case Manager to join our team! The Case Manager is responsible for performing a wide variety of client care activities as directed by the Executive and Clinical Director. He/she collaborates with the Treatment Team to formulate the Master Treatment Plan, assists in coordinating discharge planning, provides support to clients and Clinical Team, and could have the responsibility for maintaining communication with the referral source and other services provided as needed.


Job Duties:

  • Review assessments from referring facilities and note problems/issues that require further attention
  • Contact referral source and other key individuals to gather additional information as necessary; Maintain contact with referral source and other key individuals
  • Review initial treatment plan, including case management issues and be sure that client and staff assignments and responsibilities are understood and in progress
  • Formulate a comprehensive treatment plan with client that effectively addresses client issues, as it pertains to continuing care, and is completed within established time frames as it pertains to continuing care.
  • Document the client’s progress accurately as it relates to case management
  • Continue treatment planning process throughout client’s stay in treatment, as it pertains to medical, legal, and employment issues, opening new problems and developing or revising treatment plans as necessary to assist in continuing care.
  • Provides case management to clients at least one (1) times per week or more frequently if needed
  • Assist clients in understand treatment goals as it pertains to continuing care plan and link client with internal and external resources as needed to meet client needs
  • Maintain regular contact and open communication with families and assist them in finding resources to help themselves
  • Assist client in managing outside stressors (legal, financial, vocational, etc.) according to the best interests of the client’s progress in recovery
  • Prepare client to maintain recovery post-discharge and develop a discharge plan that meets client needs for ongoing support

Qualifications and Requirements:

  • Bachelor’s Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or a related field; OR a Bachelor’s Degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice or other related social sciences; OR an equivalent combination of experience and training required
  • Current licensure by DOS or be certified Allied Addiction Practitioner (CAAP) or Certified Associate Addiction Counselor (CAAC), Certified Alcohol and Drug Counselor (CADC), and Certified Advanced Alcohol and Drug Counselor (CAADC) in good standing at the time of hire OR be actively working towards licensure or certification through the PCB, once hired.
  • 2-4 years of work experience in substance abuse and mental health field preferred
  • CPR card
  • Experience in conflict resolution
  • Compassionate and caring demeanor
  • Ability to build rapport with clients
  • Excellent written and verbal communication skills
  • Familiar with community resources
  • Experience with KIPU EMR preferred
  • Experience working with veterans preferred.
