Firefly Children & Family Alliance is looking for a full or part-time Home-based Case Manager to provide community services within Greene County. You will be based out of your home office, but provide services to clients in their home or community within Greene County. You will need to provide services to individuals, children & families on a routine basis. These services include parenting education, evidence-based practice curriculum, support and transportation. You will support your clients by attending court, responding to crisis and stabilizing family functioning.
Firefly is currently expanding its Home Based program! To support this expansion Firefly is currently offering a hiring bonus of 2000 for this position. This position is also qualified for our Student Loan Repayment program which pays up to 4000 per year towards student loan debt. (Conditions apply. Full-time only. Lifetime maximum student loan benefit of 40,000)
You Will:
- (Family Progress - Monitoring and Coaching (50%)
- Coach collaboratively – maintaining a balance between teaching and serving
- Visit each youth/family on a routine and 'pop in' schedule in the client homes & community
- Assist youth/families in achieving their goals
- Provide education and support to youth/families
- Provide transportation to youth/families
- Attend court with clients and render recommendations
- Communicate with schools and multiple institutions related to youth/family matters
- Respond to all crisis calls and manage those situations
Service Plans / Goal Development (20%)
- Create treatment plans, including goals, for each youth/family
- Update plans and goals
- Develop relationships with agencies in the community
- Work with other social welfare agencies
Intake and Needs Assessment (15%)
- Determine 'crisis' situations
- Visit each youth/family to gather necessary information to complete the assessment
- Follow Firefly Children & Family Alliance professional standards
Report (15%)
- Maintain case and agency paperwork according to standards
- Prepare written reports for the Department of Child Services / Probation office within identified timeframes (Add 4-5 key responsibilities of the job))
You Have:
- Bachelor's degree in social work or related field is required
- Must be at least 21 years of age
- A valid driver's license and insurance with no more than 6 points on driving record
- Travel between appointments to service clients within their community and in their homes
- Knowledgeable regarding social service systems within the state
- Knowledge of and adherence to current confidentiality practices
- Appreciation for and ability to respond to cultural differences
- Develop relevant networks of contacts (List required professional experience)

You will be provided home office equipment to perform job adequately without needing to travel to a regional office. Your reporting director will be located in Terre Haute; majority of supervisions and trainings will be performed virtually.
