CASE MANAGER Job at VisionQuest National LTD
POSITION SUMMARY
The Case Manager is responsible for assisting in the day to day operations of the Case Management Team including assessing and responding to the needs of youth and families in the program. This position develops Individual Service Plans, screens for human trafficking concerns, and facilitates the safe and timely release or discharge of youth from the program. The Case Manager works closely with related departments and documents the provision of services.
ESSENTIAL FUNCTIONS:

Ensures enrollment in public school, verifies that medical needs are up to date and that the youth is added to the internal system.

Conducts initial intake interview of youth including; sponsorship information, gathering familial information, seeking available options for youth and maintaining contact with youth’s family members, friends, and sponsors.

Ensures a timely completion of assessments: Initial Intake, Baseline Assessments, and Monthly Speciality Court Summaries in accordance with VQ, county, and state requirements. Additional assessments may be required.

Coordinates and transports youth to legal/court appearances when necessary.

Submits a Significant Incident Report within 24 hours upon notification to immediate supervisor and ensures appropriate documentation in our electronic records system (e.g., Extended Reach) .
Documents actions taken with youth, referral source (e.g., probation officer), family, and stakeholders in the form of progress notes and monthly summary reports as required by VQ, county, and state requirements.
SKILLS & ABILITIES:

Excellent verbal and written communication skills.

Organized with attention to detail.

Ability to work independently and as a member of a team.
Knowledge of computer skills, including Microsoft Software programs.
EDUCATION & EXPERIENCE:

Bachelor’s degree in the behavioral sciences, human services or social services field.
Child welfare, probation, and/or case management experience is preferred.
QUALIFICATIONS:

Must be 21 years of age or older.

Required to work a flexible schedule (evenings, weekends, holidays).

Must have a valid driver’s license.

Cleared background checks from appropriate entities.

Must pass a drug test and physical.
Able to obtain an FBI Level 1 clearance card, child abuse clearances, and state police background check.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

May lift and carry up to 10 pounds frequently, up to 20 pounds occasionally, up to 50 pounds infrequently.

Performs tasks that may involve standing or walking 50% of the day; sitting 50%of the day.

May bend, stoop, reach and grasp as necessary.
