CASE MANAGER

Job Description
Case Manager will provide counseling and certain case management services to clients in a residential setting. They will be responsible for documenting all actions according to State and Federal requirements.

DUTIES:

Conduct intake evaluations and interviews, prepare and maintain individual case records including but not limited to individual program plans, histories, progress notes, discharge summaries, etc.

Counsel residents on an individual basis, conduct family counseling sessions and conduct group sessions according to program outlines and guidelines.

Observe client behaviors in order to make recommendations and carry out treatment needs.

Prepare and submit pass requests for residents on the caseload.

Other duties as assigned and needed.

After 60 days, health, vision and dental insurances are offered and a 401k retirement program.

QUALIFICATIONS AND EXPERIENCE:

Case Manager will have a Bachelor's degree or above in social work, psychology, or a related field. Experience is preferred, but not required.
