Case Manager for Kentucky Moms Program - Summary

NorthKey Community Care is seeking a full-time KY Moms Case Manager. The KY Moms Maternal Assistance Towards Recovery (MATR) case manager provides support to expectant mothers to find resources and behavioral health treatment to reduce stress and risk factors during pregnancy. These resources and support may include financial resources, housing resources, nutritional information, transportation, and behavioral health treatment. The successful candidate will have a Bachelor’s degree with at least one year experience in a human services field. This position also requires advancement within 2 years as a Certified Prevention Specialist or Certified Alcohol and Drug Counselor.

Case Manager for Kentucky Moms Program - Qualifications

  • Bachelor's degree or above in a behavioral health and/or human services field of study from an accredited school is required.
  • At least one year experience working with individuals with a history of mental illness and/or substance use.
  • Excellent written and verbal communication skills.
  • Requires advancement within 2 years as a Certified Prevention Specialist or Certified Alcohol and Drug Counselor.
  • Valid driver's license and the ability to operate a motor vehicle.
  • May and December 2022 graduates encouraged to apply.

Case Manager for Kentucky Moms Program - Job Duties

  • Provide behavioral health care coordination services to clients, specifically expectant mothers, who are experiencing symptoms of mental illness and/or substance use as part of a multi-disciplinary team of behavioral health professionals.
  • Participate in staffing meetings and consultation with other clinicians to assess care coordination needs of clients and encourage appropriate referrals to internal and external community services.
  • Assess, plan, implement, and document care coordination services to assigned clients.
  • Act as an advocate to clients to ensure they receive necessary services in the community.
  • Develop knowledge and establish good working relationships with community support resources and services within the community.
  • Maintain communication and positive multi-disciplinary working relationships with clinicians and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations.
  • Complete timely and accurate documentation within an electronic health record and other documentation as required of the position.
  • Attend and participate in relevant continuing education and in-service training to improve professional competency and clinical intervention skills, specifically as it relates to veterans of the United States military.
  • Maintain flexible hours to meet organizational and client needs.
