Job Description
POSITION SUMMARY
The Case Manager is a Registered Nurse who develops an ongoing interactive and supportive relationship with patients in order to provide assistance in coordinating their healthcare needs and to assist them in identifying their own individual health goals. The RN Case Manager makes home visits to assess the patient’s knowledge base, daily routine and health status. Determines available community resources and ensures patients receive the right services at the right level of care. Serves as the patient advocate by communicating to providers, agencies and other facilities to address patient care as the patient’s condition requires. Incorporates education into the care plan for patients and family members. Processes Community Care Network (CCN) referrals and reviews for potential enrollment with the Director and members of the interdisciplinary team. Trains and mentors CCN Volunteer Health Coaches and attends hospital case management meetings as directed.
POSITION QUALIFICATIONS
Education/Licensure/Certification:

Current State of California Registered Nursing (RN) License required.

Current Basic Life Support (BLS) Certification preferred.

- Must meet all DMV requirements listed below and have adequate transportation.
Knowledge:
-
Graduate from a professional school of Nursing with at least one year of experience as a professional nurse required.

Previous Case Management and/or home care experience preferred.

Knowledge of community resources and healthcare financial systems required.

Experience where leadership, integrity, initiative and effective communication skills have been demonstrated.
Skills:

Must exhibit flexibility, adaptability and willingness to change or try new approaches.

Must have the physical and mental health adequate to meet the demands of the job.

Must be self-directed with good organizational skills.

Must have strong listening, coaching, communication (written and verbal), collaboration and interpersonal skills.

Ability to work independently and effectively as a member of an interdisciplinary team.
- Note: Requires employees to drive at least once per pay period. Candidates must possess and supply proof of: a current CA Driver’s License, a clean DMV printout and Auto Insurance coverage with a minimum of $50,000 per person/$100,000 per occurrence (or a combined single limit of $100,000) and property damage coverage in an amount not less than $50,000 per occurrence.
