Job Description:
Davis Behavioral Health is actively recruiting for a full-time Case Manager/Social Service Worker (SSW) to assist adult clients with severe and prolonged mental illness to work at regular jobs of their choosing.
The successful candidate will spend 60% of his/her time in the community developing jobs for clients. Marketing and/or job development experience is strongly preferred to aid with this task. Due to the independent nature of this position when in the community, applicants need to be self-motivated, organized, and energetic. DBH utilized the Individual Placement and Support (IPS) model to assist clients with their employment.
Primary Job Duties
IPS supported employment and supported education helps people with severe mental illness work at regular jobs and education of their choosing. Although variations of supported employment exist, IPS refers to the evidence-based practice of supported employment and supported education.
The IPS Case Manager will be responsible for completing the following essential job functions:

  • Providing supportive life coaching to clients; employment and education supports.
  • Providing coverage to clients as needed in the program.
  • Providing on-site job coaching to client as needed to maintain successful Supported Employment, and Independent Employment
  • Participate in IPS training.
  • Adhere to IPS philosophy
  • Other responsibilities as assigned by Program Director or Supervisor.

Employment Schedule
This position is classified as full-time, non-exempt.
The proposed schedule for this position is outlined below:

  • Monday through Friday with primarily daytime hours. Schedule requires flexibility to meet with community resources to develop and strengthen professional relationships.

Compensation
Base Rate

  • $17.00 per hour for candidates without a bachelors degree
  • $18.00 per hour for candidates with a bachelors degree
  • $21.00 per hour + 1% per year of licensure as an active SSW for the first 10 years and an additional 1% for each 3-year period thereafter

Required Experience:

Candidates licensed as a Social Service Worker (SSW) or a Licensed Practical Nurse (LPN) in the State of Utah are strongly preferred. Candidates who are not currently licensed but are actively pursuing SSW licensure or those that do not have a SSW license, but are eligible for Targeted Case Management Certification may also be considered.
Targeted Case Management Certification
Candidates who do not hold a professional license must complete certification as a Targeted Case Manager (TCM) within the first 30 days of employment.
Qualifications to become a certified Targeted Case Manager through the State of Utah Division of Substance Abuse and Mental Health include:

  • 400 hours of work, volunteer, and/or education experience in human services or a related field
  • Required 40-hour case management practicum and examination provided by DBH during the TCM certification class.

Preference will also be given to candidates who are bilingual English/Spanish.
The successful candidate must also have a clean driving record to operate agency vehicles and/or transport clients, when necessary.
Davis Behavioral Health utilizes an electronic medical record, which requires proficiency in computer-related skills.
Meeting monthly face-to-face service expectations and productivity standards is also required.

The successful candidate must be able to pass a pre-employment background screening, drug screening, and have a clean driving record.
The ideal candidate will have experience working with clients that have sever mental illness, are hard working, can work both independently and successfully within a small outreach team.
