Rate of pay: $30.00/hr
Service Excellence
1. Collaborates with the interdisciplinary team to ensure a positive outcome upon discharge.
2. Patient Experience of Care: Address client concerns as soon as possible and handle them accordingly. 3. Ensures care is coordinated within Meridian and external agencies caring for our patients.4. Surveys reflect a positive experience and satisfaction with care.5. Adheres to all regulations regarding confidentiality and protection of health information.6. Provides activities that are coping skill development oriented.7. Provides information/education to clients regarding health and health goals.
Stewardship

- Service Volume at or above target
- Case Management/Intervention = 1173 billed hours during the fiscal year
- Responsible for meeting weekly and yearly productivity expectations
- Expected to bill for 6 hours daily unless discussed with supervisor
- A minimum of 4 clients should be scheduled on calendar at least 2 weeks in advance.
- Consistently seeks approval for overtime
- Manages workload to include documentation within shift
- The Nurse protects the assets of the Center, including its reputation, by immediately reporting any adverse event, or potentially adverse event, involving a person served, Center property, fellow employee, or community stakeholder.
- Requests time off with appropriate notice (15 days) and notifies the team lead as soon as possible if calling off for an emergency or illness.
6. Focus on building and maintaining positive working relationships with Meridian stakeholder, internal and external.
7. Demonstrates a commitment to Meridian through maximization of resources, attention to financial effects of actions, and dedication to providing service to all appropriate patients.
Practice Excellence
- Ensure working safely and effectively in collaboration with clients.
- Complete monthly home visits.
- Document failed attempts in the clinical record.
- Make sure client quotes are included in notes and assignment.
2. Documentation supports medical necessity.
3. Documentation supports use of appropriate interventions, including S/R avoidance.4. Contributes to required therapeutic activities and patient care.5. Works collaboratively with team to ensure services are provided in accordance with Meridian's procedures, State and Federal laws and community standards.6. Participates in opportunities for program growth and/or improvement.7. Ensures concurrency in documentation.8. Assesses and collects data to determine client stability, safety and status on admission. Integrates assessment findings with previously reported or documented information to determine changes in the client's status. Recognizes interrelationships between assessment findings, medications, and lab findings. Follows up abnormal assessment findings, changes in status, and lack of response to therapy 9. Implements care based on assessment of client status, treatment plan goals and progress toward desired outcomes.10. Initiates, verifies and caries out physician orders transcribing to the Medication Administration Record as appropriate.11. Collaborates with other members of the team to assess progress toward goals. 12. Administers medication, tuberculin skin testing, and provides wound care as ordered by a physician.13. Ensures that necessary safety precautions are taken based on the client's condition and facility policy.

- Provide an educational plan weekly for supervisor's approval that will be provided to clients such as (personal hygiene, nutrition, medications, managing weight etc.)
- Works effectively with others. Willingly mentors new staff and offers leadership when asked. The RN Takes initiative in solving problems and addressing needs.
- Attends the mandatory CAT team Meetings weekly.
- Reviews and submits Datis time sheet every 2 weeks by 8am (non-paid Fridays).
·Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements·In order to qualify as a Center Driver, the candidate must at least 21 years of age and have an active Florida Driver's license with no driving limitations.·Staff must use Center Vehicle when available. Use of personal vehicle for Meridian business approved only if attempt to secure Center vehicle is made.·Motor Vehicle Record will be reviewed for eligibility·No more than 3 moving violations and/or accidents within the past 3 years and no major violations during the past 5 years, and/or 1 serious traffic conviction.·Drivers over 65 years old must provide a current medical note stating they have no driving limitations.
