POSITION SUMMARY:

EPIC Rehabilitation and Nursing at White Plains is a **BRAND NEW** Skilled Nursing & Rehabilitation center located in the heart of White Plains. This SNF is part of our " Epic Healthcare Management" group of facilities.

  • Follow work assignments, and/or work schedules in completing and performing assigned task. Perform all tasks in accordance with established policies and procedures, and as instructed by supervisors.
  • Notify the facility when you will be late or absent from work, according to facility policy.
  • Cooperate with inter-departmental personnel, as well as other facility personnel to assure that nursing services can be adequately maintained to meet the needs of the residents.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
  • Participate in and receive the nursing report upon reporting for duty.
  • Observe resident’s skin closely during care and report the presence of pressure/red areas or areas of breakdowns.
  • Provide daily perineal care. Provide daily range of motion exercises.
  • Turn bedfast residents at least every two hours.
  • Provide residents with reality orientation as instructed.
  • Prepare residents for meals such as take to bathroom, wash hands, raise bed, position tables, place bibs, and take to dining room.
  • Serve food trays. Assist with feeding as indicated and needed. Monitor diet order and diet served for errors.
  • Assist residents with sight problems to identify food arrangements on tray. Record the resident’s food/fluid intake. Report changes in the resident are eating habits. Assist resident to receive alternative tray if resident does not like the food served or does not eat it.
  • Keep resident water pitchers clean and filled with fresh water (on each shift) and within easy reach of the resident.
  • Perform after meal care such as removing trays, cleaning resident’s hands, face, and clothing.
  • Monitor diet orders and restrictions, and make sure resident does not receive foods they are restricted from having due to medical reasons or due to allergic reactions.
Requirements:
  • Minimum High School Diploma (or equivalent).
  • Valid Certification as a Nursing Assistant in the State of New York, in good standing.
  • Excellent communication and organization skills
  • Speak English in an understandable manner. Knowledge of other languages may be helpful.
  • Ability to work well under pressure and be flexible.
  • Able to relate to and work with the ill, disabled, elderly, emotionally upset, and sometimes challenging individuals within the facility.
  • Have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents
  • Have the ability and willingness to work harmoniously with professional and non-professional personnel
  • Nursing home experience preferred, not required.
