Client Service Coordinator
Hourly Rate: $15 - $17
Status: Full-time, hourly, non-exempt
Location: Temple Hills, MD
Empowering Minds is a human services agency dedicated to improving the social functioning of adults, adolescents, and children within a framework that recognizes the primary importance of family and community. To this end, the agency’s mission to provide comprehensive mental health care to families and children utilizing the services of qualified professionals implementing individualized rehabilitation plans that assist all persons in obtaining their desired goals. To honor this commitment, we are involved in partnering with other agencies aimed at providing better health care. These include consultation and service provisions to area schools, other community agencies, groups and individuals, as well as other cooperative prevention efforts.

Job Summary:

The Client Service Coordinator provides client support services to our PRP and Care Cooridination programs.

Duties:

  • Submit authorizations through OPTUM Maryland INCEDO
  • Ensure compliance with COMAR 10.21.21 and 10.21.29, contract terms, policies, and procedures.
  • Work cooperatively with family and professionals to coordinate services with other agencies and programs in the best interest of the client.
  • Receive and process referrals
  • Assign Intakes to Intake Coordinators for completion
  • Review completed intakes and annual assessments for accuracy
  • Follow up with DSC’s and clients to support, engage and connect clients
  • As needed handling of client crisis, issues, questions, and concerns
  • Assign, receive, track and monitor Rehabilitation Plans
  • Contact via telephone and interest letter and/or assign interim visits to client’s with “On Hold” status to encourage re-engagement
  • Collaborate with therapists to ensure client therapy compliance
  • Process DLA-20 document for client concurrent authorization
  • Read monthly contact notes from Direct Service Coordinators for consistency and accuracy
  • Process client discharges to include discharge notification letter, discharge process in Beacon Health Options and Discharge Summary preparation
  • Other duties as assigned

Minimum Qualifications:
AA degree preferably in Human Services field
One year of experience within the mental health/social services/human services field
Strong organizational skills
Excellent time management skills

Job Type: Full-time

Pay: $15.00 - $17.00 per hour
