Travel Requirements: Primary location of SVT Health and Wellness is in Homer, Alaska. This position is expected to report directly to our Homer location before travel by automobile to Anchor Point. Once this position and processes are more established, it is expected that this candidate may be able to report directly to Anchor Point.


Schedule: Monday through Friday, 40 hours per week.

Position Overview:
Prepare patients for their office visit to include obtaining and documenting vital signs and medical history. Provide assistance and support to the professional health care provider. Champion recommended vaccines in order to not miss any opportunity to vaccinate. Perform lab duties as required. Cross-train with Patient Service Representatives for essential job functions.


Duties and Responsibilities

  • Work directly with Clinicians to support clinical visits. This includes assistance with all phases of the visit cycle: pre-visit preparation, visit, post-visit activities, and follow-up home visits as required. Additionally, the LPN performs requested activities to assist in population health management, quality improvement, and quality assurance activities.
  • Transport vaccines in order to have relevant recommended adult vaccines on hand with the goal of not missing an opportunity to vaccinate. This may include transporting vaccine(s) to patient homes if needed to reduce barriers to vaccination as determined by care team.
  • Understand and apply principles of aseptic technique and "Universal Precautions" for infection control
  • Maintain strict confidentiality for all clients and their families and health center information in compliance with HIPAA. Protect the security of all patient medical/ dental records to ensure that confidentiality is maintained following HIPAA and PPI regulations as outlined in SVTHW policy and procedures.
  • Prepare patient for examinations, procedures and treatments: Obtain chief complaint, basic screening tests, and vital signs
  • Assist with examinations, procedures, and treatments as required
  • Recognize and respond to emergencies
  • Coordinate patient care information with health care providers. Sends letters to patients with laboratory, radiology and procedure results as directed
  • Communicate effectively with patients and co-workers
  • Clean, stock and maintain patient care areas and laboratory ensuring inventory is maintained
  • Clean, package, and sterilize supplies and instruments
  • Collect and process laboratory specimens and CLIA waved diagnostic tests
  • Clean and maintain equipment. Cleaning duties as required to include: patient areas, lobby and laboratory
  • Perform requirement for OSHA and CLIA compliance such as refrigerator temperatures, laboratory quality assurance tests, autoclave spore tests, etc.
  • Perform basic clerical functions.
  • Participate in orientation of new medical assistant staff
  • Assist in making patient referrals and patient follow-up as directed. Obtains medical records from legacy systems or outside providers such as ER notes as needed. Manages miscellaneous medical records such as: psychiatric visit notes from outside providers and medical records received for non-established patients.
  • Conduct patient education on recommended vaccines, health promotion, chronic conditions and self-care, and medications based on the provider’s plan of care at clinic visits and at home visits as needed.
  • Conduct community presentations/trainings on a variety of health education topics with particular emphasis on vaccine education and education on topics to address vaccine hesitancy.
  • Perform focused assessments of specific client health problems.
  • May start IV’s and monitor infusions upon competency check off.
  • Be proficient with Competencies and Skillsets as outlined for work performed.
  • Project a professional manner and image. Adhere to ethical principles.
  • Actively participate in one of the SVTHW committees as assigned by Management. This may require acting as Chair or Co-Chair of the assigned committee.

Job Requirements

Education: High School Diploma, Required

Experience: 2+ years experience in direct patient care, preferred. Clinical setting preferred.

Certifications/Licensure: Current Alaska LPN license. Phlebotomy Certification, preferred. CPR Certification, required – provided by SVT.

Computer Skills: Proficient in Mac/PC. Ability to perform clinical computing requirements to include: electronic medical record (EMR), telepharmacy, telemedicine to include data entry, store and forward, digital and video photography, inventory control, ordering, email, internet, MS Excel and Word

Travel: Travel required up to 30-45% of the time is required between Anchorage, Homer, Seldovia, and Anchor Point by car, boat, or plane. Overnight travel is required.
