We are in search of a Clinical Care Coordinator to supervise, provide and coordinate nursing care in compliance with facility and regulatory policies and procedures and to assess residents’ care needs.

Earnings:

• RN: Starting Rate: $38.50 / hour and as high as $44.50 / hour. Starting rate increases based on years of experience working as an RN
• LPN: Starting Rate: $34.25 / hour and as high as $39.25 / hour. Starting rate increases based on years of experience working as an LPN

Qualified candidates will have Charge Nurse experience and be a Registered Nurse or Licensed Practical Nurse licensed in the State of Michigan. Excellent written and oral communication skills, be able to display leadership through staff education and training.

Primary functions include:

• Interview residents, residents’ representatives and others with information related to the Minimum Data Set (MDS) assessment.
• Collect all data necessary to complete MDS forms and related documentation.
• Make appropriate referrals to other health care professionals.
• Participate and/or conduct interdisciplinary meetings.
• Supervise and participate in the overall care of the residents.
• Oversee clinical coordination, review labs, assist the physician with rounds.
• Participate in the facilities Quality Assurance process to review facility systems as directed.
