Community House Mental Health is looking to fill the position of Mental Health Case Manager, at the Jackson Outpatient Clinic.


Job Details:


Full Time: Mon-Fri 9:00am to 4:00pm (35 hour work week)


Full medical, dental, disability, 25 plus days off per year to start.


Note: COVID -19 Vaccination Required


Job Summary:


  • Works as part of a team to provide outpatient mental health services and community support in a clubhouse setting.
  • Is assigned a caseload of 50-55 clients.
  • Writes treatment plans and reviews in a collaborative format with clients identifying goals and methods to reach goals.
  • Writes and keeps progress notes per agency guidelines.
  • Serves as focal point for crisis intervention.

Essential Job Duties:


  • Provide immediate crisis intervention and resolution as needed.
  • When necessary, supervise members taking medications, visually making sure they are taking correct doses at appropriate times.
  • Maintain member and staff confidentiality at all times.
  • Make sure all incoming agency telephone calls are answered.
  • Have legible handwriting.
  • Be able to do simple mathematics.
  • Be able to read and comprehend agency/member charts.

Specific Clinical/ Support Duties and Responsibilities:


  • Provide comprehensive therapeutic community support to an assigned caseload. Treatment includes identifying and using community resources, liaison with other member treatment providers, referral to other appropriate community programs, and life skills training.
  • Advocate for member needs which include but are not limited to assistance with Social Security Disability Office, Department of Vocational Rehabilitation, landlords, housing authorities, hospitals, medical clinics, and police.
  • Maintain member records in keeping with agency, county, and state guidelines.
  • Evaluate treatment effectiveness with agency medical/clinical team for voluntary and involuntary clients. This includes revision of all treatment documentation as required by the WAC and per supervision direction.
  • Outreach to member homes to assess living situation, stability, and provide treatment interventions when appropriate.
  • Facilitate assigned group therapy sessions and specialized member events i.e. field trip, Christmas party, etc.
  • Provide individual treatment sessions when appropriate to member needs and per prescribed treatment planning.
  • Assist in discharge planning and written summaries of treatment as needed for member referral and releases of information.
  • Perform assigned intakes and presentation to clinical team.
  • Perform assigned milieu therapy/clubhouse duties.
  • Attend scheduled meetings and conferences, including at least two hours per week (per FTE) with Clinical Director and/or Psychiatrist.
  • Some crisis after hours work is included in the specific responsibilities for this position.
  • Other duties as assigned.

Job Requirements:


  • Current counselor registration/certification.
  • Current First Aid and CPR cards.
  • Current home phone/address.
  • Valid driver’s license and clean driving record in order to use agency vehicles.
  • Documentation of personal insurance in order to use personal vehicle for member transport and to use agency vehicles.
  • 24 hours per year FTE of continuing education/trainings.
  • Food Handler’s Permit.
  • Agree to permit State Patrol Background Check.
  • Be free of substance abuse.
  • A working knowledge and adherence to all agency policies and procedures.

Minimum Qualifications: This position requires a Bachelor’s degree in social sciences, or related degree, and at least 6 months of clinical experience working with target population.


Experience Required:
Experience working with people who have mental illness or related social service experience and/or ability to acquire specialized training to work with target population preferred.


Skills Required:
Ability to work independently and within a team.


Salary:
$24.15Hourly/and up DOE and Education level
