The San Francisco Community Health Center Wellness Clinic provides primary medical, behavioral, and dental care. While serving residents across San Francisco and the Bay Area, the Wellness Clinic aims to provide robust, comprehensive, and integrated care to individuals who are at the highest risk of disengaging from care. As a result of this effort we provide full primary care in our clinic and on the street.
Position Summary:
The Clinical Program Manager will provide day-to-day oversight, supervision, and coordination of the Wellness Clinic case management Team, and is also responsible for providing social work and behavioral health services to the agency’s clients who are trying to manage their mental health and substance use. This position requires a graduate level degree with active licensure.
Clinical Duties

  • Conduct initial intake and regular assessment and reassessment of clients with mental health and substance use disorders accessing the program
  • Conduct acuity assessments and reassessments of all clients referred to the program
  • Offer individual psychotherapy for clients referred to the program based on need
  • Provide outreach and navigation assistance to clients in the program on an as- needed basis
  • Maintain a small caseload of clients with high behavioral needs
  • Develop and utilize multiple modalities to address clients’ substance use and mental health needs, including but not limited to individual psychotherapy using evidence-based treatments
  • Continually update client care plans and develop appropriate discharge plans
  • Provide service referrals, linkages, care coordination, and advocacy services for clients, and monitor the outcomes of these activities
  • Provide referrals to medical and psychosocial service resources and programs when needed
  • Provide onsite consultation for the medical clinic and behavioral programs for urgent situations
  • Complete necessary documentation and reports on units of service (UOS) and treatment plans in a timely manner; for third-party payers, thoroughly document visits for coding and billing
  • Lead and participate in case conferences, monthly staff meetings and in-service trainings
  • Participate in and comply with agency quality assurance, peer review, clinical consultation.
  • Work collaboratively with an interdisciplinary team of medical services, medical case management, prevention and testing services, and other internal and external social support programs
  • Take after-hours and weekend on-call coverage and crisis intervention, as needed

Programmatic Duties

  • Serve as day-to-day contact person for all program staff
  • Review program objectives and performance and design strategies, as needed, to improve service delivery
  • Ensure that all direct service staff provide strengths-based, trauma-informed, and culturally competent services to clients
  • Deliver or arrange training and resources required for direct reports and direct service staff to perform their roles
  • Ensure program deliverables are being met
  • Develop and maintain protocols to ensure client engagement in care
  • Develop protocols that ensure seamless integration between both programs
  • Work with collaborating agencies to ensure strong partnerships are maintained
  • Develop, maintain, and support schedule of SFCHC staff, including (1) requests for home visits/outreach needs and (2) open access programming
  • Attend and facilitate case conferences weekly for their program
  • Hire, supervise, evaluate and develop direct reports
  • Assist with other administrative duties as needed

Minimum Qualifications:

  • Master's degree in social work
  • Active licensure with the California Board of Behavioral Sciences
  • Two years of program management or project management experience
  • Two years of direct staff supervision
  • Two years of post-Master’s direct clinical expertise in assessing and managing the psychosocial needs of low-income populations, multiply-diagnosed patients (HIV, mental disorder and substance abuse), sexual and gender minorities, immigrants and refugees
  • Two years of experience with harm reduction, client-centered counseling, motivational interviewing, and other evidence-based treatment approaches
  • Two years of experience working in interdisciplinary teams. Must be able to work independently and also have an adaptable working style and be a strong contributor to team performance
  • Two years of experience developing and facilitating psychotherapy and support groups
  • Experience working with diverse populations including BIPOC, LGBTQ+ communities, refugees/immigrants, people living with HIV / AIDS, people experiencing homelessness and multiple diagnosed populations (substance use and/or behavioral health issues)
  • Demonstrated reliability and good interpersonal, organizational, verbal, written communication skills. Must be able to handle multiple tasks and have ability to work professionally and ethically within multi-racial and multicultural settings
  • Be able to provide customer service in an effective, courteous and friendly manner and meet the needs of diverse clientele
  • Knowledge of legal mandates and ethical issues related to client privacy and confidentiality
  • Ability to travel within San Francisco and, at times, the Bay Area or nationally
  • Fully vaccinated against COVID-19 and able to show proof of vaccination

Preferred Qualifications:

  • Five years of experience working in an acute setting and conducting individual and group psychotherapy
  • Familiarity with data systems such as ARIES, electronic health record systems, and Medi-Cal documentation requirements
  • Fluency in English and Spanish
