Your Role

As the Clinical Specialist with Evergreen Nephrology, you will educate, develop, support, and identify ways to improve our proactive clinical care teams and thereby help to ensure optimal clinical quality for our patients. You will collaborate with internal and external stakeholders in the implementation of our strategic direction and advancement across the kidney care continuum. You will exemplify the definition of clinical standards and support the implementation and auditing of what great care looks like. You will assess the current state and needs of our care teams, compare that to the vision of where we are going, and assist building and executing on the clinical and quality plan for closing identified gaps. You will utilize a teamwork approach to build a culture of clinical excellence to achieve top performance of clinicians. You will engage deeply with internal and external stakeholders and partners to understand and align incentives, processes, and operational execution. By doing so, you will help drive significant improvements in clinical quality and outcomes, cost effectiveness, consistence in care delivery and model fidelity in our interdisciplinary team approach.

Primary functions for the Clinical Specialist include:

  • Assist in the development and support the clinical quality program; exemplify the definition of great care and clinical standards in kidney care while developing and supporting care teams.
  • Partner with clinical leadership, Evergreen Academy and training team to roll out clinical education that supports the quality program goals and standards.
  • Utilize technology and provide feedback to the population health operations team to ensure clinical excellence as we operationalize clinical pathways, workflows, and processes; communicate goals set to our care teams and provide education and mentorship to support clinical excellence to drive high quality and reliability in care.
  • Build relationships with our nephrology partners and care teams, gather feedback, and help identify improvement opportunities to assist in the design of pathways to eliminate barriers to care and achieve optimal clinical quality.
  • Coach, train, and mentor members of our care teams who have not delivered kidney care and/or who have not delivered value-based care in a similar complex care management model.

How You Qualify

  • You hold an unrestricted RN or NP state license.
  • You have 3-5+ year of clinical practice experience in nephrology.
  • You have proven experience leading care teams focused on clinical quality and outcomes for care delivery and/or managed care organizations.
  • You have experience educating team members in various roles and work groups with conviction, decisiveness, and positivity.
  • You have strong comprehension of clinical standards of care and are focused on quality of life for our patients.
  • Bonus points: You have a master's degree of higher related to education, quality, public health, business administration, health services research or a related field
