Position Summary:
The Clinical Staff Pharmacist in conjunction with the Director of Pharmacy and pharmacy leadership in accordance with established policies and procedures, participates in and verifies the medication use process in both the inpatient and outpatient setting to provide the safest and highest quality care for our patients.
Additional responsibilities include but are not limited to:
- Clinically review medication orders to ensure patients are receiving medication therapy according to the most up to date clinical standards
- Provide therapeutic drug monitoring and follow up for patients
- Educate patients and caregivers on proper medication therapy management
- Collaborate with other healthcare professionals
- Compound, dispense, and verify medications, including sterile compounding
- Opportunities available for precepting both IPPE and APPE students
- Support continuous improvement and growth of pharmacy practice
- Solves practical and strategic problems
Minimum Qualifications:
- Registered pharmacist in good standing with the state of Connecticut,
- B.S. or PharmD. degree
- Completion of an ASHP-accredited pharmacy practice residency or at least 2 years of experience in a hospital pharmacy setting
- Good patient, provider and staff communication skills
- Strong commitment to working with patients and staff from a wide range of ethnic, economic, cultural and social backgrounds
- Strong interpersonal skills including the ability to handle internal and external issues in a calm, positive, and professional manner
- Ability to maintain a professional demeanor and confidentiality
Preferred Qualifications:
- Board Certification in Pharmacotherapy (BCPS) or other specialty
