Position: Community Ambassador

Program/Department: MAPP Monkey Pox Awareness & Prevention Partnership

Work Location: 132 32nd Street, Brooklyn, NY

Full Time: 35 hours per week

Salary: $45,000 - $45,000

Position Summary:

Under the supervision of the Program Coordinator, the Community Ambassador will primarily be working out in the community with the MSM target populations. The Community Ambassador will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. Employees will be a part of a team working to disseminate information about the Monkey Pox Virus (MPV) and linking clients to MPV vaccination services.

Responsibilities:

  • Convey the purposes and services of the program to the assigned client population

  • Assist and guide clients in utilizing MPV services, information, and scheduling appointments for vaccination

  • Attend training and workshops with DOHMH

  • Assist client in understanding MPV services and provide updated information about MPV via phone, text, Grindr, email, and social media

  • Document activities, collecting data relating to service engagement, and linkages to vaccination. and follow through with the work plan provided by the program director while adhering to the policies and procedures in place.

  • Facilitate communication and coordinate services between client and community health resources

  • Continuously expand knowledge and understanding of MPV and services available

  • Follow-up with health management/care plans with both patients and providers

  • Establish positive, supportive relationships with participants and provide resources and supportive engagement and education to allow positive outcomes.

  • Motivate patients to be active, engaged participants in their health
  • Build and maintain positive working relationships with the clients, providers, nurse case
  • managers, agency representatives, supervisors and office staff
  • Perform other related duties as assigned

Qualifications Required:

  • Customer relation experience, HS Diploma/ GED Equivalent
  • Completion of the Community Health Worker certificate preferred
  • Demonstrated ability to manage time and projects and submit work in a complete, on time and accurate manner
  • Interpersonal skills necessary for teamwork, decision-making and management and to work collaboratively and effectively within a team
  • Demonstrated ability to respond with sensitivity and confidentiality in compliance with HIPAA and other regulations
  • Demonstrated ability to effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
  • Familiarity with Motivational Interviewing approach helpful
