Description:

The full-time Community Based Prevention Health Coordinator reports to the Project SHINE Department Manager and works closely with the Community Based Prevention Program educator and Prevention and Screening team. The right candidate for this position provides in person and virtual outreach, risk assessment, risk-reduction education, implements CDC evidence-based Personalized Cognitive Counseling (PCP) curriculum, and links patients to PrEP and behavioral health services. The Bilingual Based Prevention Health Coordinator leads CBP HIV and STDs prevention initiatives, and has a focus in the Latinx gay, bisexual and other men who have sex with men.

Schedule: 9am-6pm, and may include evening and weekend rotations

Duties and Responsibilities:

  • Under the direction of the Director, leads the PCC program to accomplish its objectives and goals.
  • Works collaboratively with EBNHC’s Marketing Department and helps coordinate marketing efforts.
  • Responsible for data collection, reports, monitoring and evaluation of program.
  • Works closely with EBNHC’s Grants Department to ensure appropriate and timely expenditure tracking, and grant compliance.
  • Ensures the goals of the PCC program are met on a consistent basis.
  • The provision of direct patient care, including assessment, supported referrals to PrEP services and behavioral health services.
  • Follow up and oversight for special high-risk populations. This includes coordination of services with outside behavioral health providers when necessary and periodic outreach to engage patients in care.
  • Ensures program documentation, reporting, evaluation and overall compliance.
  • Maintain confidentiality of patient records.
  • Develop and sustain relationships with local agencies in East Boston.
  • Ability to work collaboratively with multiple community and city-wide stakeholders.
  • Ability to work through brief patient contacts, and make quick and accurate clinical assessments of mental and behavioral health conditions.

Requirements:

  • Bilingual (English/Spanish) required.
  • Bachelor’s Degree in either Arts or Science Preferred; Master’s Degree in related field (public health, health and/or human services, social work related) preferred.
  • Minimum of 1 year relevant experience, preferably in a community health, mental health or hospital setting.
  • Comfortable with the pace of primary care and emergency care, within with an interdisciplinary team.
  • Willingness to work with the guidelines within assigned behavioral interventions, such as Personal Cognitive Counseling and more.
  • Ability to work independently and collaboratively, and demonstrate sound judgment.
  • Comfortable with facilitating groups and using social media
  • Willing to provide feedback and receive feedback.
  • Program Development; Strong Written and Verbal Communication Skills
