Community Development Services Leader Job at Caroline Center, Inc.
- Coordination of community activities, for groups not to exceed 4 Participants, based on Participants’ goals and desired frequencies. Identify greater-community resources and activities that are personally meaningful to the Participants included in each activity
- Create schedules to include identification of Participants who will benefit from activities.Identify how each Participant will benefit from activities Establish start and end time of each activity
- With oversight of DSC, addresses plans for personal care needs and dedicated support hours.Ensures adequate vehicles available for planned supports in addition to healthcare appointments and other program needs.Develops and submits weekly calendar of activities to the DSC by 8:30a every Thursday
Plans to include:
Up-coming week’s activities Monday – Friday
Individuals who will participate in each activity
Specific timeframes for each activity
How the activities relate to Participants’ goals
Provide Personal Support Services as assigned
Assist with Day Hab. Services when assigned
Provide 1:1 CDS when assigned
Participate in a variety of team meetings, and as assigned
Participate in the Leader On-call rotation and as assigned

OVERVIEW:
- 18 yrs+ AND no longer enrolled in primary or secondary school.Skills development/maintenance for community engagement/integration- community based, with people who do not have disabilities
- Increase independence in the community.Promote positive growth and develop skills that will support gaining, retaining or advancing social networks or work toward competitive employment
- Learning socially acceptable behaviors & self-advocacy skills. Activities that facilitate and promote integration and inclusion in communities of their choice
- Self-advocacy classes & activities,Community Events
- Volunteering and time-limited generic paid and unpaid internships for the development of employment skills; and time-limited participation in “Project Search” or similar programs approved by DDA.
- Provided in a variety of community settings; Setting should not have institutional qualities, should not isolate or segregate (disabled from non-disabled). IF THE PARTICIPANT CHOOSES A DISABILITY-SPECIFIC GROUP OR ACTIVITY, IT MUST BE DOCUMENTED IN THE PCP.Development of an individualized schedule- estimate of what, where and when; up-dated as indicated based on needs, desires and circumstances
INCLUDES:
●Direct support- can be “virtual” with all criteria met; enable participant to learn, develop and maintain general skills related to participation in the community VIRTUAL must be added to PSP.
●Transportation to, from & within (provide or provide for to meet needs per PCP)
●Delegated Nursing Tasks as assessed/needed- as a separate service based on HRST; can change if circumstances change
●Personal Care Assistance- based on need and cannot represent the entirety of the service
●Eligible for 1:1 and 2:1 if assessed based on assessed need and approved by DDA
●Distinct from residential and cannot bill simultaneously
●May return home only for personal care or behavioral needs that cannot be met in the community; must be indicated in PCP
CANNOT:
♦Exceed staffing ratio of 1:4; unless approved to participate in a time-limited internship
♦Receive stand-alone “Transportation Services” simultaneously
♦Transportation cannot comprise the entirety of service
♦Delegated nursing supports cannot comprise the entirety of the service
♦Exceed 40 hr/wk or 8 hr/day, or be provided on Saturdays or Sundays; as a single service or in combination with Day Hab.
♦Combine with other Meaningful Day Services on the same date, UNLESS funded through LTSS and authorized in the PCP
