Shift: 40hrs, Days, schedule varies and could include weekend and evening shifts
Location: UMass Memorial-Marlborough Hospital with local community travel
UMass Memorial-Marlborough Hospital strives to be the community hospital of choice for patients and the workplace of choice for health care professionals and physicians.
Summary: Conducts culturally sensitive community education programs for identified at-risk patients and their families to improve overall health outcomes in the community. This role would require the employee to be in the community about 50% and in the office 50%. This includes, but is not limited to, health fairs, screening programs, educational seminars, coordinate community programs, works with Mass Health and other health programs. Works closely with the clinical team and provides reinforcement of education and instructions, referral to community resources, and ongoing communication.
Qualifications: Equivalent to high school education and some relevant college courses required Associate degree or bachelor’s degree in related field preferred. Minimum one (1) year of experience in a clinical office or community outreach setting is required. Previous experience working with diverse populations is required. Must have familiarity with local community. Excellent oral and written communication skills is required. Must be Bilingual (Portuguese or Spanish/English). Ability to work independently, yet function as part of a larger team is required Computer skills is required. Ability to work evenings and weekends as needed is required. Reliable transportation with a valid MA Driver’s license is required OR equivalent combination of the above
