The Community Health Worker provides case management services including in-home counseling, education, referrals, and advocacy to families with lead poisoned children in the Massachusetts Merrimack Valley area.
The responsibility of the MVLPPP Community Health Worker are:
- Conducts home visits
- Completes client intakes and makes appropriate service referrals.
- Collaborates with the MDPH Childhood Lead Poisoning Prevention Program (CLPPP) staff/team members to ensure optimal case management
- Maintains timely and accurate case records
- Attends regularly scheduled case reviews, meetings, and trainings
- Participates in data gathering for monthly and quarterly reporting
- Assists with community education/outreach as needed
- Performs other related duties as requested by the Program Director
Skills & Qualifications:
- Associate’s Degree in related field and/or two or more years of work experience required
- Prior home visiting, community advocacy, and/or outreach experience is strongly preferred
- Must have basic computer skills including proficiency in Microsoft Word, Outlook, PowerPoint
- Excellent interpersonal and verbal skills.
- Ability to work with diverse population.
- Refined and well-organized multi-tasking skills.
- Bilingual/bicultural (English/Spanish) preferred.
- Use of own car as means of transportation.
- Must have a suitable CORI and SORI completed within the first thirty (30) days of employment.

Physical Demands: This job regularly requires the employee to sit, walk and stand as well as use hands repetitively to handle or operate standard equipment. This employee is occasionally required to kneel, crouch, or crawl. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
Work Environment: This job operates in a professional office environment and uses standard office equipment, such as computers, phones, photocopiers, filing cabinets, fax machines.
