Community Health Worker
Employment Type: Full-time / 40 hours
Job Location: Muskegon, MI
(Must travel within Muskegon to Northern Ottawa and Oceana Counties)
Hackley Community Care (HCC) is partnering with Senior Resources to provide mental health services to the senior or homebound residents in the community. HCC is looking for a Community Health Worker (CHW) to serve as a link between the seniors/homebound person and mental health therapist. This CHW will assist the person with setting up virtual visits with the therapist, setting follow-up appointments, and coordinating with Senior Resources and HCC.

Job Responsibilities:
- Uses the protocols for care coordination.
- Interviews clients, enters information from home visits into database, advocates for client and acts as a liaison between client and other service providers.
- Chart encounters in the EMR for Hackley Community Care patients encounters.
Job Qualifications:
- High school diploma or equivalent.
- MICHWA certification required.
- Excellent interpersonal and customer service skills to effectively and tactfully deal with a diverse group of patients, staff, customers and community groups.
- Strong oral and written communication skills.
- Detail oriented and excellent organizational and time management skills.
