POSITION SUMMARY: The Community Health Worker (CHW) offers care coordination through the
King County Care Connect hub through specific authorized services. These services are designed to
provide State authorized clients who have tested positive for COVID-19 with resources they may need
during quarantine and/or isolation. The CHW must demonstrate competence in providing culturally
competent, non-judgmental care to their communities while working remotely. Some travel is required to
pickup and deliver supplies in safe and coordinated manner from the King County Care Connect Hub

(HealthierHere) to clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Convey the purposes and services of the program to the user population and the impact that program or service would have on their health through effective means of outreach.
  • Provide culturally appropriate case management services to qualified clients who are facing isolation.
  • Assess and address other needs relating to the social determinants of health within the scope and guidelines of the program.
  • Conduct virtual visits through phone, teleconference and coordinate the delivery of services and goods according to the program requirements.
  • Ensure appropriate documentation and gather outcome data under the direction of the Program Director and/or Program Supervisor.
  • Develop and maintain relationships with the Care Connect Hub, HealthierHere.
  • Work effectively with community members, staff, partners and other CHWs to reduce cultural and socio-economic barriers.
  • Identify gaps in health services and report to the Program Director and/or Supervisor
  • Communicate information to clients and providers clearly and succinctly, both written and verbal.
  • Requires strict adherence of HIPAA standards in the office workspace to ensure confidentiality.
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each

essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Minimum Qualifications

  • Bachelor’s degree preferred, High School Diploma or equivalent work experience in relevant field of case management, care coordination or social services.
  • Minimum of 2 year working in a case management or social service role.
Preferred Qualifications

  • CHW Certification is preferred but not required.
