1st $500.00: after 30 days of employment

2nd $500.00: after 6 months of employment

Essential Duties:

• 
  Promotes positive customer service culture which encourages quality patient outcomes and promotes staff retention

• Completes SAMMS intake assessments for new patients

• Completes an individualized treatment plan for each patient on caseload, addressing all needs identified in the psychosocial assessment

• Provides group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives

• Properly documents all services provided in individualized progress notes per DASA, Joint Commission, and FGC regulations, standards, and policies, summarizing the patient’s status and ongoing treatment needs

• Reviews and evaluates patients’ progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans. Modify treatment plans to comply with changes in patient status

• Plans and assists with the implementation of aftercare and continuing recovery plans once patients are discharged from treatment programs

• Works with patient and other staff, when necessary, to deescalate and assist with patient emergencies and crisis situation

• Coordinates care with legal, law enforcement, and treatment professionals, both within and outside the agency, to help ensure patient needs are met and increase positive treatment outcomes. This would include Will County representatives, Safe Passage program contacts, and others

• Makes appropriate patient referrals to outside agencies for the provision of ancillary services

• Assist with the collection of toxicology specimens, as needed
  

• Attend in-house and off-site trainings for continuing education, in order to expand knowledge, improve the quality of patient services, and to maintain licensure and/or certification at all times

• Ability to use calendar and scheduling software, email, and agency’s clinical software program. Experience using desktop computers, copier, and fax machine

• Other duties as assigned

Schedule:Monday - Friday (9:00a - 5:30p) with one night to 6:30p for group coverage

Required Experience:

Education and/or Experience: Associate degree (or higher) and prior substance abuse treatment experience preferred. Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply

Certification, Licensure, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) required or able to obtain within allotted time of hire date
