Hopewell Health Centers is a nationally accredited federally qualified health and community mental health center with 23 sites across TEN counties throughout Southeastern Ohio(Athens, Franklin, Gallia, Hocking, Jackson, Meigs, Perry, Ross, Vinton, and Washington). We are currently seeking a CPST Provider to join our Crisis Stabilization Unit located in Gallia, OH team.

CPST Provider Schedule:

  • 2pm-10pm
  • Monday-Friday (with flexibility as needed)

CPST Provider Responsibilities:

  • Provides case management services on the CSU within the scope of licensure/certification and in accordance with all applicable policies, rules, and standards including service coordination with other case managers.
  • Conducts evaluation/assessment of client’s strengths, needs, abilities, and preferences. Administers substance abuse screening tool and determines appropriate level of care. Participants in the development of the client’s ISP with goals and objectives in the client’s own words
  • Monitors residents’ ability to self-medicate, assists clients in learning their medication regime.
  • Possesses knowledge and expertise in de-escalating disruptive clients.
  • Responds to individual crises, as needed and makes the appropriate referrals.
  • Answers Crisis Hotline calls and provides telephone support to callers in crisis, including but not limited to: active support, directing callers to local resources based on individual needs, and contacting on-call staff to dispatch to various locations.
  • Assists and participates with the client in obtaining and maintaining essential community resources including housing.
  • Participates in, coordinates, and maintains systems for discharge planning and/or transition from inpatient, residential, or from one outpatient service or provider to another.
  • Maintains minimum acceptable productivity expectation as designated by agency and supervisor directives. Follows billing procedures as required by agency policy.
  • Completes correctly and submits in a timely manner per agency policy documentation, billing, time sheets, mileage, and, as required by supervisor(s) work and appointment schedules.
  • Informs supervisor(s), client rights officer, and/or human resource director, as appropriate, of any complaints from clients, families, members of the community, or workforce members of actual or suspected workforce member abuse/harassment towards clients or their family/significant others or other workforce members.
  • Attends as scheduled and participates in supervision, team meetings, professional development, First Aid/CPR training and serves on agency committees, as appropriate.
  • Abides by agency code of conduct, policy and procedures and mission statement (e.g., business, clinical, personnel, and plan manuals) at all times on agency property or while conducting agency business.
  • Performs other related duties as assigned by supervisor(s), program director, clinic director, and agency administration.

CPST Provider Requirements:

  • Eligible to work in the United Stated
  • High School diploma or equivalent (Bachelor’s Degree in a related field, LSW, D&A experience and/or Adult Mental Health field preferred)
  • Experience in direct care Case Management and/or working with persons with mental illness or similar population.
  • Good verbal and written communication skills
  • The ability to work as part of a multi-disciplinary team and to be sensitive to the cultural needs of the clients
  • A valid driver's license, good driving record and vehicle suitable for transportation.
