About Your Role:

Pathways of Idaho is looking for a dynamic Crisis Center Case Manger to join our team. The Clinical Crisis Specialist is responsible for maintaining functions within the scope of practice as established by the contract developed through the Department of Health and Welfare regarding the Pathways Community Crisis Center of Southwest of Idaho. The Crisis Case Manager is responsible for maintaining functions within the scope of practice as established by the contract developed through the Department of Health and Welfare regarding the Pathways Community Crisis Center of Southwest Idaho.

Qualifications:
  • Bachelor’s Degree
  • Must pass fingerprint clearance (and maintain during employment) and be able to pass the background check.
  • Preferred degree in a behavioral health field. Must pass a criminal background check conducted by the DHW.


Essential Duties and Responsibilities:
  • Works with adult clients experiencing a behavioral health crisis to provide support and encouragement during client’s difficult moments.
  • Maintains confidentiality of all client documents, files, and communication in written, electronic, and/ or verbal form.
  • Documents all client interactions within the WITS electronic health records system.
  • Assists adult clients on achieving their personal recovery goals.
  • Provides individualized assistance and education in obtaining community services, maintaining life supports such as housing or income, carrying out the tasks of daily living, and participating in support groups or other community networks.
  • Offers education and support in working toward physical and mental wellness, including healthy living behaviors.
  • Serve as a resource to the Pathways Crisis Center team in incorporating recovery strategies and tools into client interactions, reinforcing an approach of client self-determination in all aspects of service planning.
  • Participate in any in-service training for Team members.
  • Completes intakes, discharges, and aids in resources for client in rotation with peer support, or as needed.
  • Provides introduction into the facility to aid client in understanding the process, expectations, and admission/ discharge steps in rotation with peer support (i.e. must see the therapist and case manager at admission and prior to discharge).
  • Aids in completion of Wellness Plan prior to client’s discharge, as necessary, unless client is at extremely high risk for suicide or homicide (therapist does it in this instance).
  • Talks with client throughout the admission process when client is not in with other disciplines and not with peer support.
  • Engages in team activities for the client and in order to meet client needs such as, but not limited to, completing team staffings after every client’s admission and at the start of every therapist’s shift, providing for client with regard to creating a welcoming atmosphere, and maintaining a clean and positive environment (i.e. picking up after a client whom nursing staff puts in the shower, giving meals and cleaning up leftovers after the client eats, completing laundry and other chores).
  • Monitors / observes client for safety such as checking on clients every minute when client is in the bathrooms, observing clients in the day and sleeping areas, talking with clients in between assessment and discharge, providing discharge plans and resources to the client prior to discharge.
  • Participate in any in-service training for team members.
  • Notifies staff designated per shift/team responsible for inventory of any medical supply needs.
  • Writes client information on triage board and identifies discharge plan
  • Completes other duties as assigned or necessary to support the program/ company.
