Critical Time Intervention Case Manager Job at Valley Community Services Board
VCSB is looking for a Critical Time Intervention (CTI) Case Manager (CM)! The CTI CM will provide intensive, time-limited, community mental health case management to Permanent Supportive Housing (PSH) participants who are transitioning from hospitalization or homelessness to permanent housing.

Summary of Essential Functions
- Provides comprehensive initial and annual assessment of client strengths and needs.
- Participates in development of the consumer service plan.
- Refers, links, and coordinates Medicaid and non-Medicaid services.
- Advocates and promotes recovery oriented practice.
- Provides counseling and case management services.
- Provides crisis intervention and crisis stabilization services as needed.
Minimum Qualifications
All applicants must meet the following minimum qualifications in order to be considered qualified for the position:
- QMHP certification,
- One year of clinical experience with adults experiencing serious mental illness, including developing, conducting, and approving assessments and individual service/treatment plans, and
- Valid Virginia driver’s license with an acceptable driving record.
