Child & Family Services is looking to hire a full-time Family Partner for the Community Service Agency (CSA) Program at our Acushnet Avenue location! The Family Partner is responsible for providing support and advocacy to parents/caregivers, and facilitation of peer support groups for caregivers. This individual is directly responsible to the Director of the Community Service Agency.
**Candidates who are bilingual in Spanish, Portuguese and/or Cape Verdean Creole are highly encouraged to apply!
QUALIFICATIONS:
- High School Diploma/Associates Degree with 2 years of professional experience working with youth and families or Bachelor’s degree with 1 year of professional experience.
- Must have lived experienced as an adult who is the parent/caregiver of a youth with an SED and preferably a youth with mental health needs.
- Must have an understanding of systems of care, be able to work collaboratively with a Team, and possess effective interpersonal skills and communications skills.
- Must have a valid driver’s license and reliable transportation
- Availability during evening/weekend hours is required.
RESPONSIBILITIES:
The following responsibilities are not meant to be all inclusive and may be adjusted to meet the agency’s needs.
- Engage the parent/caregiver in home and community resources designed to address one or more goals on the youth’s treatment plan or ICP
- Assist the family in understanding the scope and services of the CSA and the family’s role and responsibilities in the program
- Teach parent/caregiver how to network/link and secure necessary community resources and treatment providers
- Participate in child’s Individual Care Planning Team (ICP by developing clear and outlined goals
- Maintain regular contacts with parents/caregivers and youth set and agreed at the ICP
- Participate in the child’s Individual Care Planning Team (ICP) by modeling and educating families in how to access community resources
- Deliver strength-based, culturally and linguistically appropriate services under the supervision of a Sr. Family Partner
- Provide in home and in community mentoring and education to the parents/caregivers in order to enhance problem-solving, parenting effectiveness, and communication
- Be available to parents/caregivers by phone or at the family home during non-business hours as needed
- Advocate parent/caregiver about the education system by attending school meetings and participating in the development of any school plans/IEP’s; and within other systems such as the courts, state agencies and community organizations
- Monitor the family’s satisfaction with the CSA
- Participate in the systems of care committee meeting
- Perform other duties as required
