Day Habilitation Specialist

Good Home Community Living currently has a day habilitation program serving adults with developmental disabilities (consumers). Good Home Community is built around consumers’ Individual Service Plans, their desired community engagement outcome goals, and providing an optimal level of quality and care to their lives while they attend our program. Our center is open from 8 AM– 4 PM, Monday through Friday. During program, we have classes to further our consumer’s education such as: math, reading, writing, and spelling. Our curriculum has many exciting and engaging activities focused on recreation and community inclusion. Due to Covid-19 our consumers are stationed at specific locations within the Good Home Community Living centers. Transportation for consumers will begin at 7:30am

We are seeking a qualified and caring individual to join our team!

Day Habilitation Support Staff Duties:

• Transportation: All support staff have a route with assigned consumers they will transport in the morning to day program and home from day program. Routes usually start around 7:30 AM and most end by 4:30 PM. Support Staff are also required to drive to and from activities during the day.
• Make sure each client is signed in by transportation staff in the morning and signed out in the evening.
• Give medications as prescribed daily and document medication sheets. Ensure that all paperwork is documented daily for each client by end of shift.
• Maintain proper inventory of supplies by supplying a list bi-weekly to supervisor or management.
• Conduct monthly fire drills
• Clean facility throughout the day to ensure a safe and healthy environment. Assist clients with their personal hygiene throughout the day.
• Active Supervision: Support staff are required to supervise consumers while staying engaged and participating in activities.
• Personal Care: some of our consumers need assistance toileting, eating, walking, and transitioning on and off transportation.
• Ensure and safeguard individuals' human dignity, rights.
• Provide each client, according to their level of comprehension the assistance needed in developing skills in education, social interaction, and recreation activities that will enhance their life daily.
• Make sure all clients lunch is provided from group home staff when clients come in the morning. Also, make sure all clients have a morning and afternoon snack.
• Regularity lift and/or transfer/move Individuals in and out wheelchairs, vehicles up from and down to a mat etc.
• Escorting consumers to off-site doctors appointments, escort consumers on all outside breaks,

7:30am-4:30pm Day Habilitation Male (preferred) w/Drivers license

4pm-12am Mon-Friday

12am-8am Mon-Friday

At Least One Weekend a Month is Required

8am-8pm Sat-Sun Only

All shifts must have driver license!!

Experience of 1year required

Must Pass Background check and Drug Test Must have a Valid Drivers License

QUALIFICATIONS:

Must have transportation, must be willing to work weekends minimum one weekend a month

Paid training is provided.

To ensure we keep all staff and cilents safe we wear face mask/shileds and gloves during any encouters with cilents and outsiders. We also check tempature daily clients and staff all have been tested for covid-19 an free of virus.
