The Mary T Family of Companies is seeking a Designated Coordinator to oversee the Individualized Service Options department in the Coon Rapids - Elk River Metro area.

The Designated Coordinator oversees the operations and delivery of services in the Individualized Service Options department. Individualized Service Options offers customized support options to individuals with disabilities in the local community through a collaborative, person-centered approach. The responsibilities of this position involve the management of the staff and clients, including but not limited to: daily staffing, performance management, client programming, client service authorizations and staff training.

The ideal candidate will be team-oriented, possess good communication skills, provide positive customer service support, and have the ability to work independently. Excellent time management will be required. The ability to lead annual meetings for persons served will be ideal.


SCHEDULE:

This is a full-time position, typically daytime hours Monday-Friday 9 AM - 5 PM, however flexibility is required to cover evening and weekend shifts as needed.


ESSENTIAL FUNCTIONS AND BASIS JOB DUTIES

  • Manages and supervises Community Support Specialists, including developing and conducting ongoing training, holding monthly meetings, enforcing company policies and procedures, monitoring and documenting staff performance, conducting performance evaluations, completing and reviewing time cards and mileage/expense forms, and billing audits.
  • Manages and oversees activities related to client programs for individuals served, including reviewing and maintaining outcomes and goals, developing person-centered plans, completing new intake and annual paperwork, and communicating with the Support Team (support staff, case manager, guardian) as needed.
  • On an as-needed basis completes the following activities:
    • Teaches independence and assists in the coordination of activities of daily living, such as shopping, budgeting, computer skills, vocational training and searches, supporting hobbies/interests, household chores, medical appointments, community activities, etc.
    • Completes documentation including TLOGS, client program, Scomms, incident reports, monthly billing forms, time cards, and service authorizations.
    • Work direct care with individuals in the community
  • Ensures training requirements are met, including monthly assigned CDS requirements, monthly staff meetings and annual training.
  • Participates in the Individualized Services Options Emergency Cell Rotation and provide support and direction to team members in those areas.


BASIC QUALIFICATIONS FOR THIS POSITION

  • EDUCATION:
    • High school diploma with 4 years of relevant prior experience, or
    • Associates degree with 2 years of relevant prior experience, or
    • Bachelor’s degree in Human Services or a related field and 0-1 years of relevant prior experience.
  • CERTIFICATION/LICENSURE:
    • Valid driver’s license
  • EXPERIENCE:
    • A minimum of 1 year of management experience in unit-based services and/or Individualized Home Supports.
    • Experience with 245D paperwork, person-centered goal planning and writing outcomes.
    • Group home management experience is a plus.

KNOWLEDGE, SKILLS, ABILITIES:

  • Working knowledge of industry regulations pertaining to Vulnerable Adult reporting and licensing requirements.
  • Knowledge of community resources and of Minnesota Department of Health and Minnesota Department of Human Services regulation and standards.
  • Knowledge and skill in the principles of person-centered thinking and knowledge of client rights.
  • Proficiency in commonly used programs such as Therap, ADP, Work Schedule and Google.
  • Ability to prioritize and self-direct.
  • Strong problem-solving skills and ability to organize information and ability to work in ambiguous situations.
  • Knowledge of Positive Behavior Supports and ability to manage conflict.
  • Ability to establish rapport with persons served, getting to know how they think and feel, and the individual needs of the person served.
  • Intermediate to advanced computer skills.
  • Strong verbal and written communication skills.
  • Strong organizational skills.
  • Ability to hold staff accountable to organizational expectations.
  • Knowledge of service authorizations and billing procedures.
  • Ability to work independently with high level of self-motivation
  • Must be flexible and able to adapt to change
