The Direct Care Professional is responsible for promoting a supportive, structured relationship for Individuals with Disabilities, which fosters Individual development and self-sufficiency; and enriches the Individual’s quality of life. The DC professional is responsible for maintaining a safe, supportive, home environment which facilitates the development of self-help skills among Individuals.
The primary role is to provide the appropriate supports tailored to the needs and desires of each Individual to enable each to live as independently as possible in accordance with the Assessment and ISP. The DC Professional is expected to recognize when an individual is in a crisis and be able to utilize the appropriate behavioral and medical support services. The DC Professional will demonstrate a commitment to promoting normalcy, in the scheduling of and participation in activities, vocational training, and job goals; as well as daily home management routines that enable Individuals to lead productive and fulfilling lives.
The DC Professional will follow the predetermined Assessment and ISP as set forth by the team and ensure that all aspects of the plan are carried out. This Professional will demonstrate a commitment to enrichment of the Individual’s lives and the parameters and guidelines as set forth in the Assessment and ISP. This position reports to the Community House Manager.

Position Responsibilities
- Ensure that the safety and well-being of the Individuals are protected at all times and that the rights of the Individuals are observed at all times
- Assume responsibility for the Individual under your care at all times
- Compassionate, patient and caring work attitude
- Ability to follow written directions/plans and record results daily
- Implement behavior management techniques as taught by a certified trainer or behavior therapist, when needed or applicable
- Supervise Individuals’ Health care, including the administration of medications and transportation to appointments
- Encourage and support the Individual’s’ membership and interaction with community recreational facilities
- Attend trainings and meetings as required
- Strong team player
- Attention to detail and accuracy
- Ability to adapt to spontaneous situations
Education and Experience Requirements
- High School Diploma or equivalent, valid Driver’s License and a minimum of two years working with people in a customer service, health/human services or business environment
- Ability to read and understand English as it relates to reading medical prescriptions, directions, recipes, policies, procedures and directions for managing the daily life and activities of Individuals and a safe and healthy home
- Proven communication skills both written and verbal
- Demonstrated home management skills, to include, budget management, cleaning, menu preparation and nutrition and emergency steps
- Must be able to lift 40 lbs., able shovel snow and climb stairs and physically adept at doing household chores
- Successfully complete physical examination within 12 months prior to employment and every 2 years thereafter including a Mantaux Tuberculin Test/Chest X-Ray
