Your Role:
As a Direct Support Professional in Sertoma’s Community Day Services (CDS), you will provide individual and group training in a supportive environment for people with intellectual and developmental disabilities. This includes the provision of 1:1 services to specific consumers, as needed. This role is essential to Sertoma’s mission of providing compassionate care and ensuring services are delivered in a person-centered manner.
Position Responsibilities:
- Assist with activities of daily living, including meals, basic personal hygiene, and ambulation.
- Transport consumers to and from Community Day Services and/or activities.
- Facilitate group and individual curriculum and activities, in various settings, including on-site, community settings, home settings, and/or virtual settings.
- Provide and document individualized services in accordance with the personal planning process.
- Complete all required documentation in a timely manner to ensure adequate record keeping.
- Ensure delivery and/or coordination of Community Day Services are in compliance with DHS Rule 119, CARF standards, agency mission, agency policy & procedure, program guidelines, and best practices.
Job Qualifications:
- High school diploma required.
- Minimum of one (1) year of experience working with individuals with disabilities preferred; ability to learn and work effectively with persons with various types of disabilities required.
- Ability to follow written and oral instructions and to work cooperatively and effectively with consumers, families, team members, inter-disciplinary team, and representatives of referring organizations.
- Effective oral and written communication, math skills, organizational, and computer skills.
- Ability to provide accurate reports and data collection.
- Valid driver's license with good driving record, if applicable; completion and maintenance of PACE certification, if eligible.
- Proficiency in the use of computers, software applications, and working knowledge of Microsoft Word and Outlook preferred; willingness to learn necessary software applications required.
- Completion of DSP certification within four (4) months of hire.
